As a client of this practice, you have the right to request access to or copies of your health care records that we maintain.

To request your records:

• Please submit a written request by email to info@ceptherapy.com
• Include your full name, date of birth, and signature
• Records will be provided in accordance with Texas and federal law, including HIPAA
• Reasonable fees may apply as permitted by law
